launched its newest INNitiative, the Center for Transformative Change (CTC), a unique facility located immediately adjacent to the Mary Brennan INN soup kitchen in Hempstead. CTC serves as a Resource Center to assist guests of the soup kitchen to improve their life circumstances and\/or become self-sufficient.\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-28951b4 optimal-tstd-medium elementor-widget elementor-widget-heading\" help in locating and securing affordable housing (NOTE: Very few landlords are accepting applications or new tenants at this time)<\/li><li><strong>Id Documents<\/strong>: Assistance in applying for birth certificates, driver and non-driver licenses, Social Security cards<\/li><li><strong>Veteran Assisstance<\/strong>: Helping contact Services for the Underserved<\/li><li><strong>Health Insurance Enrollment<\/strong>: Help contacting health insurance representatives, (usually we have onsite access to reps, currently just phone contacts)<\/li><li><strong>Health Care<\/strong>: Referrals to health, dental and mental health services<\/li><li><strong>Mental Health<\/strong>: Onsite visits with Central Nassau Guidance counselors, tele-medicine appointments with therapists<\/li><li><strong>Furniture<\/strong><strong>:\u00a0<\/strong>Referrals to St. Vincent DePaul.<\/li><li><strong>Notary Services<\/strong>: By
